The Gated Pipeline

Move work through named stages with a named accountable human at each gate, so that every outcome has a person who can answer for it and a record that shows why.

Context

Arises wherever the cost of one wrong outcome exceeds the cost of a thousand slow ones: defense acquisition, clinical trials, credit adjudication, anything a regulator can subpoena. The default shape of regulated work, and the one no popular framework describes respectfully.

Forces

What pulls against what. A shape is a way of resolving these, not a way of removing them.

  • Throughput against traceability
  • Speed of learning against the stability an auditor needs
  • Named accountability against the tendency of named people to become rubber stamps
Structure

Work moves in lanes through fixed stages. Each stage ends at a gate held by a named person with the authority to pass, return, or halt. The record of each gate decision is produced as a byproduct of the decision, not assembled afterward. Agents may work inside a stage; they do not hold a gate.

This is the shape the human oversight duty assumes when it says understand, intervene, and halt. Mastery loads because a gate holder has to be qualified to refuse. Flow and Learning starve on purpose: the gates are slow, and changing a stage means changing the record it produces.

The gate holder
A named person with the authority and the qualification to pass or return the work. Accountable by name.
The stage team
Does the work between gates. May be human, agent, or mixed.
The record
Produced at each gate: what was decided, by whom, on what evidence. The artifact an auditor asks for.
The auditor
Internal or external. The shape only holds if someone actually asks for the record.
Preconditions

Each of these is written to be tested. If one is false, the shape is being described, not run.

01An auditor exists who asks for the record, and has done so in the last year.
02Every gate has a named holder with sized queue time, and the queue is measured.
03The cost of one error demonstrably exceeds the cost of the delay the gates add.
04Gate holders are qualified to refuse, and have refused something in living memory.
Consequences

When it works, you give up

  • Throughput. The gates are the point, and they are slow.
  • Learning speed. Changing a stage means changing the record it produces, and that needs approval.
  • Talent that wants to move fast. The shape selects for people who value being able to answer.

How it breaks

  • Throughput death, and shadow processes routed around the gate.
  • Gate holders approve blind when the queue gets noisy, and the record shows a signature without a decision.
